Ask the Medical Staff: Sports and Intramurals
The North Hunterdon Voorhees School District utilizes the platform Aspen. Aspen is a data management system that will hold all of your student’s academic and health information as they progress through High School. This is also where students will sign up for all after school activities including after school sports, clubs, and intramurals.
To register your student for sports, intramurals, or marching band, log into your parent Aspen account and click on Page Directory on the left-hand side. Click on North Sport to fill out the required sports registration forms, which are explained below. For help navigating the North Sport section in Aspen, please click here for assistance.
To participate in Athletics at North, you will need to complete the following EACH Sports Season:
- Complete the Consent for Athletic Participation Form (This form is completed in the Parents Aspen account and must be completed for every sports season. Click here for further assistance)
- Complete the Health History Update Form (This form is completed in the Parents Aspen account and must be completed for every sports season. Click here for further assistance)
- Complete the Sports Wellness Acknowledgement Form (This form is completed in the STUDENTS Aspen account and must be completed for every sports season. Athletes will click on Page Directory on the left-hand side, then click on NH Student, and complete the Sports Wellness Acknowledgement Form. In the form, athletes will review the Sudden Cardiac Arrest pamphlet from NJ Department of Education and the Opioid Video from NJSIAA. Make sure they click Submit at the bottom of the form so the form goes through)
To participate in Athletics at North, you will need to complete the following ONCE per year:
- PPE Medical Eligibility Form – **IMPORTANT**: The State of New Jersey has changed its requirements surrounding the need for student physicals. For participation in Athletics/Activities, the Nurses office does NOT need your child’s full physical report. We MUST receive the above linked one-page document signed by your physician and dated within ONE YEAR OF THE START DATE for your sport/activity in order to clear your child for participation!

- **Please understand that simply handing in your paperwork to the school does not constitute clearance. New Jersey State Law, still requires our school doctor to sign off on your PPE Medical Eligibility Form before granting clearance**
- IF your child needs an INHALER FORM, EPI-PEN FORM or DIABETIC MANAGEMENT PLAN FORM you need to have the corresponding Health forms on file with the Health Office for your child to be cleared for sports.
To participate in Athletics at North, you will need to complete the following once every TWO years:
ImPact Test – The ImPact test (concussion baseline test) will be completed online. All registered athletes will be sent a personalized link to their school email to complete the Impact test at home prior to the start of sports practice. Only new students or those whose test has expired will have to complete the Impact test (primarily freshmen and juniors)

How do I know if my Child’s PPE Medical Eligibility Form is still valid?
All PPE forms must be dated within one calendar year of the start date of the sport/activity.
For example: Soccer tryouts/start date is set for August 19th.
- Player A’s PPE expires on August 16th – Player A must submit a new PPE Medical Eligibility Form to the health office.
- Player B’s physical expires August 27th – Player B will be allowed to participate in Fall Sports.
For example: Basketball tryouts/start date is set for November 25th.
- Player A’s PPE expires on November 24th – Player A must submit a new PPE Medical Eligibility Form to the health office.
- Player B’s physical expires on December 4th – Player B will be allowed to participate in Winter Sports.
For example: Track and Field tryouts/start date is set for March 10th.
- Player A’s PPE expires on January 24th – Player A must submit a new PPE Medical Eligibility Form to the health office.
- Player B’s physical expires on March 11th – Player B will be allowed to participate in Spring Sports.
My child’s physical/PPE Form was done on August 20th last year, and due to my insurance regulations, I cannot get him another physical until August 23rd. Will my child be cleared for tryouts???
If you are having this problem, please email the nurses. We will try to help get your child cleared for practice and understand the complexities of insurance. Specifically, he/she would NOT be clear until the new PPE Medical Eligibility Form has been cleared by our school physician. When he clears your son or daughter we will allow them to participate. This usually takes between 48-72 hours to complete but can take up to one week.
My child played a fall sport. What do I need to send in for winter team tryouts???
The parent will complete in Aspen a new Health History Update and Consent to Participate Form for that sport. In addition, the student-athlete needs to complete the Sports Wellness Acknowledgement Form for each sports season in which they participate. No other forms are needed if your child was cleared in the fall and his or her physical was done within one calendar year of the first day of tryouts.
INTRAMURALS
If my child wants to participte in intramurals what paperwork needs to be filled out?
If your child is going to participate in an intramural sport such as weight room/weightlifting program, speed and agility program, basketball, floor hockey, pickle ball, Frisbee, project adventure, etc. you will need to mirror the requirements above. The only difference is that in Aspen, parents will register their child under the “NH Intramural” section instead of the “North Sport” section. Parents will need to complete the Health History Update and Consent to Participate Form for each intramural in which they participate. In the Students Aspen account, they will need to complete the complete the Sports Wellness Acknowledgement Form. Intramural Athletes will also need to have a valid PPE Medical Eligibility Form on file with the health office.
Additionally, IF your child needs an INHALER FORM, EPI-PEN FORM or DIABETIC MANAGEMENT PLAN FORM you need to have the corresponding Health forms on file with the Health Office for your child to be cleared for intramurals.
What if my child is playing a varsity/sub-varsity sport and wants to participate in intramurals? For example my child is on the track team but is also involved in the lifting program for football, what paperwork do I need??
You need all of the requirements for their current sport on file AND to also complete the Intramural Consent to Participate Form in the parents Aspen account under NH Intramural section
