We are committed to limit the amount of paperwork that needs to be completed for this initiative. To be able to participate in this program, you only need to sign and return one form depending on your choice regarding the device as follows:
Parent-Student Device Agreement: Use this form if you are opting to use the District provided device (Samsung or Dell Chromebook). Student AND Parent/Guardian must sign this form. Form must be returned to Guidance Department at each school (Attention: Registrar).
Parent-Student BYOD Agreement: Use this form if you are opting to bring your own Chromebook. Student AND Parent/Guardian must sign this form. Form must be returned to Guidance Department at each school (Attention: Registrar).
