Attention Spring Sports Athletes –
The Voorhees Athletic Department is testing a new online process for completing sports paperwork. This process will replace the need for a paper copy of the parent permission form, random drug testing form, steroid form and concussion form. This online program through FamilyID will ultimately be easier to use and more efficient for both you and the school district than our current paper process. When you register through FamilyID, you enter your information once for multiple uses, multiple children and multiple programs.
***This program will not replace the physical examination form, which should still be turned into the health office on a yearly basis***
If you have already completed the above paperwork for the spring season, we ask that you duplicate the information online at this time. The Deadline will be Tuesday, February 26th.
A parent /guardian should register by clicking on this link https://www.familyid.com/voorhees-hs-athletics and follow these three steps:
- Click on the Spring 2013 Athletics Registration link. Scroll to bottom of page and click on Sign Up as a Family or Individual.
- Sign Up for your secure FamilyID account by entering your family name, email address and password. You will receive an email with a link to confirm your new account. (If you don’t see the email, check your spam or junk folders)
- Click on the link in your email confirmation and then fill out the information requested on the registration form.
- You will receive an email from FamilyID confirming your registration. You can log into https://www.familyid.com/voorhees-hs-athletics at any time to update your information and to check your registration(s).
